Summary: | Right atrial fungal mycetoma with extensive bilateral pulmonary dissemination is infrequent and lethal in children. The treatment approach in this situation are extensive fungal mass resection and perioperative antifungal therapy. We present a 9-year-old immunocompetent child who presented with a large right atrial fungal mass with bilateral pulmonary infiltration. Empirical oral voriconazole was given for 2Â weeks, during preparation for fungal mass resection because of rapidly deteriorating general condition. However, the outcome was fatal.
