Summary:The development of the current kidney allocation policy took almost a decade to complete and required compromise on many levels. The history of this process provides an excellent case study of how public policy is made. The final product of this process is a policy that balances utility and equity and establishes priorities for the allocation of a scarce national resource—deceased donor kidneys. While it may be imperfect, it is a dramatic improvement from the prior system and provides a framework for further revisions in the future.
