Summary:The pterional craniotomy is one of the most versatile approaches in neurosurgery. It is suited not only for vascular pathology arising from the circle of Willis but also for lesions located in or around the cavernous sinus, the sella, and the parasellar and subfrontal regions. When used with other skull base surgical adjuncts, including orbitozygomatic osteotomies and intradural and extradural removal of the clinoid, or combined with other approaches (temporal craniotomy, petrosectomy), a wider area as well as additional structures become accessible. The variations and idiosyncrasies of the pterional craniotomy are many. This article reviews the senior author's techniques for performing this approach.
