Summary: | Highlights • Folie à deux or shared psychotic disorder and Fregoli syndrome are rarely seen in clinical practice. • This remarkable case demonstrates Fregoli syndrome presenting in the context of folie à deux. • Unusually, the ‘secondary' develops a more serious illness course than the ‘primary' and reciprocally induces a de novo Fregoli delusion in the ‘primary'. • Socio-cultural factors – such as interdependent family dynamics − in addition to the classical biological and psychogenic factors may play a role in the pathogenesis of this impressive pathology. • Culturally-sensitive formulations are imperative for good treatment outcomes. |
