Summary:Custom-made sports mouthguards are believed to enhance athletes' comfort and usability during training and competitions, but their impact on oral health quality remains scarce. This study aimed to assess the impact of conventional and CADCAM mouthguards on national athletes. In this cross-sectional study involving Malaysian athletes from rugby, silat, and field hockey, participants were randomly assigned to receive either a conventional custom-made mouthguard or a CAD-CAM mouthguard. The assessment included Visual Analog Scale (VAS) ratings, Oral Health Impact Profile (OHIP-14) measurements, and mouthguard thickness. A total of 65 athletes took part in the study, with 56.8% being male and 43.2% female. Among them, 44.6% (N=29) were field hockey athletes, 36.9% (N=24) participated in rugby, and 18.5% (N=12) were involved in silat which 42 athletes received conventional custom-made mouthguards and 23 athletes received CAD-CAM mouthguards. Both types of custom-made mouthguards showed improvements in VAS and OHIP-14 scores. Athletes experienced significant enhancements in various aspects, including breathing, communication capacity, salivation, bad breath, gingival pain, tooth pain, TMJ pain, comfort, adaptation, flexibility, softness, and perceived level of protection, when comparing their experiences with previous mouthguards to the newly custom-made ones (p < 0.05). After three months, there was no notable difference in OHIP-14 scores following delivery, and changes in thickness observed in both types of custom-made mouthguards did not impact VAS and OHIP-14 scores (p > 0.05). Custom-made mouthguards play a pivotal role in maintaining oral health quality among national sports athletes.
