Summary:Frenae, lacking muscle fibers, primarily comprise dense connective tissue and elastin, contributing to dental issues. Frenectomy, a surgical procedure, entails altering the frenulum, a small tissue band connecting oral structures. Case 1: A 16-year-old adolescent in good general health presents with gingival recession associated with sulcus opening caused by the insertion of the lower frenum on the free gingiva (type II) Placeck et coll. (1974). The decision is to combine a lower labial frenectomy with a free gingival graft to restore keratinized gum tissue. Case 2: Patient, in good general conditions referred for upper labial frenectomy(TypeIII), will undergo subsequent orthodontic treatment to transform tooth 22 into 21 following the loss of 21. The decision is to perform a frenectomy combined with a triangular free gingival graft harvested from an adjacent vestibular site. Frenectomy not only enhances oral function and aesthetics but also positively affects periodontal health. The presence of a aberrant frenulum lead to gingival recession and hinder oral hygiene, while its removal improves access for thorough cleaning, reducing plaque buildup and the progression of periodontal disease. Our cases explore the efficacy of frenectomy combined with free gingival graft for treating upper and lower labial frenum. this approach being necessary to counteract the exacerbating effect of frenum traction on gingival recession. Frenectomy,a straightforward mucogingival surgical procedure, aims to reposition frenula causing direct trauma to gingival tissues. Combined with gingival graft, it enhances oral health and promotes gingival tissue regeneration
