Fatal ischemic stroke caused by cerebral small arteritis in a patient with giant cell arteritis

•A case of recurrent and fatal ischemic infarctions in a GCA patient is reported.•Multiple intracranial large-artery stenoses on MRA were found.•On autopsy, inflammations were scarce in large and medium-sized arteries.•Lymphocytic infiltrations in cerebral small arteries were observed.
