Summary:Cone beam computed tomography (CBCT) has been the most significant technological advance in terms of diagnostic imaging in the maxillofacial area. This change enabled moving from a 2D image to a multiplanar volumetric advancement of dental tissues and bone structures. The maxillary sinuses are anatomical structures that can be accurately observed. The aim of this study is to report a case of an aggressive hypodense lesion in the left maxillary sinus in an asymptomatic 64-year-old dark-skinned female patient, demonstrating the accuracy of CBCT to provide the full spectrum of the lesion such as location, dimensions, and precise involvement of adjacent structures compared to the panoramic radiographic image. The lesion measured 40 mm in the largest diameter and obliterated the entire maxillary sinus, causing destruction of all sinus cortices, maxillary tuberosity, and adjacent bony structures. The final diagnosis should be obtained by correlating the clinical-radiographic findings with the histopathologic examination.
