Summary:Osteomyelitis with proliferative periostitis, also called Garre's osteomyelitis, is a rare chronic process characterized by periosteal proliferation reactive to the presence of inflammation. A 4-year-old male patient presented with facial asymmetry on the right side with 3 months of evolution and diffuse pain. Clinical examination revealed an indurated and hyperemic facial tumor and severe trismus, in addition to a decayed second primary molar. Imaging exams revealed periosteal thickening and subperiosteal new bone formation with radiopaque lamination associated with tooth 85. Treatment involved antibiotic therapy, drainage of purulent secretions, and removal of the decayed tooth. After incisional biopsy, microscopic examination showed interconnected bone trabeculae, arranged in parallel and surrounded by osteoblasts. Multinucleated giant cells similar to osteoclasts, fibrous connective tissue with focal of lymphoplasmacytic inflammatory infiltrate, and scattered polymorphonuclear cells confirmed the diagnosis of osteomyelitis with proliferative periostitis. After 7 days there was a decrease of the edema and recovered mouth opening.
