Summary:This study aimed to report a case of a young woman (21 years old) complaining of an oral lesion for approximately 5 years. The patient reported an unusual family history, with a brother who died at age of 20 with multiple myelomas. During physical examination, a tumor was found on the hard palate, measuring 8 × 5 cm, covered by an erythematous mucosa with telangiectasia, and with a fibrous consistency. Radiographic examination revealed no signs of bone erosion. Incisional biopsy confirmed pleomorphic adenoma. Just fifteen days after the procedure, patient returned with an exophytic ulcer on the central part of the mass, covered by a necrotic area. A second biopsy was performed, now with the diagnosis of a carcinoma ex-pleomorphic adenoma. The patient underwent follow-up with partial maxillectomy and rehabilitation with bucomaxilofacial prosthesis. After two years of follow-up, the patient presented no signs of recurrence and a good general health.
