Summary:A 38-year-old man with congenital adrenal hyperplasia, salt losing variety, on a maintenance dose of 25 mg of cortisone acetate daily, presented with decreased libido and premature ejaculation, but no other constitutional symptoms. He was noted to have bilateral testicular masses. Ultrasound showed infiltrative tumors in both testicles. Laboratory testing revealed an elevated ACTH of 500 pg/ml (normal range 9-52 pg/ml) reflecting inadequate corticosteroid replacement. A I-131 6β-Iodomethyl-19-Norcholesterol (NP-59) scan demonstrated prominent testicular uptake indicating the steroid producing nature of the testicular masses.
