Summary:We report an extremely rare case of isolated fracture of the volar rim of lunate fossa which extends into the sigmoid notch, resulting in distal radioulnar joint (DRUJ) and radiocarpal joint instability. This fracture is easily missed with radiography alone in emergency setting, and should not be treated with casting due to DRUJ instability.
