Summary:It is surprising that there has been no published report concerning the effect of high dose glucocorticoid therapy in hyaline membrane disease (HMD). This therapy has been used empirically in many disorders with a much lower mortality rate than HMD. If clinical trials with either glucocorticoid or estrogen therapy are to be conducted, it is imperative that the severity of the disease in each case be assessed by objective criteria, that there be a concurrent control group matched for clinical severity and gestational age, and that currently used methods of neonatal intensive care be applied to both groups.
