Summary:We treated a total of 25 patients aged between 31 and 74 years (11 males, 14 females) by acute type A aortic dissection from May 1991 to April 1996. Patients in the acute phase underwent medical treatment and those in the chronic phase underwent surgical treatment. Surgical treatment was carried out in the acute phase in patients with serious complications (aortic regurgitation (AR) with left ventricular (LV) failure, rupture, ischemia of a major branch). Beginning in January 1994, surgical indications were extended to the acute phase. Patients with AR and an intimal flap at the aortic root, visualized by echography, were also operated on. In the group treated prior to January 1994, 3 of the 14 patients in the acute phase underwent surgery (3 for rupture).
