Practices recruited from the Ohio Chapter, American Academy of Pediatrics' database self-selected to participate in a quality improvement project. Two age-appropriate screening tools, corresponding talking points and local resources for birth-1 year and 1-5 year aged children were developed for unintentional injury and social health determinant topics.
