Summary:Anomalies of the appendix are rare, and one of the rarest is the double appendixes. Most anomalies of the appendix are observed in adults and are discovered incidentally during surgery that does not primarily involve the appendix. It is usually missed, often with life‐threatening consequences. Every surgeon must be aware of the anatomical variations of the vermiform appendix. Double appendixes should be considered in the differential diagnosis for patients with pain in the right iliac fossa. Misdiagnosis of this situation can lead to life‐threatening complications for the patient and medicolegal consequences.
