Reccurent dysphagia after redo surgery in diffuse idiophatic skeletal hyperostosis/Diffuz idiopatik iskelet hiperostozisli bir olguda tekrar ameliyatlara ragmen tekrarlayan yutma guclugu

Diffuse idiophatic skeletal hyperostosis (DISH) is characterized with calcification and ossification of soft tissues, especially enthesitis points, ligaments and the joint capsule. Dysphagia or cervical myelopathy may be seen in the cases of DISH with cervical spine involvement. In association with ossification of the posterior longitudinal ligament it can result in myelopathy and sometimes quadriplegia. Surgical interventions are rarely necessary for exceptional complications such as osteophytic dysphagia or spinal stenosis. In this article, we report a 62-year-old male patient with DISH with increased dysphagia, stiffness in neck and weakness of upper extremity for 1.5 years, who was operated three times. Despite redo surgeries, the symptoms repeated two to three months after the surgery. Keywords: Diffuse idiopathic skeletal hyperostosis; dysphagia; upper extremity weakness. Diffuz idiopatik iskelet hiperostozis (DISH) yumusak dokularin ozellikle entezit noktalarin, ligamentlerin ve eklem kapsulunun kemiklesmesi ya da kalsifikasyonu ile karakterize durumdur. Servikal omurga tutulumu ile seyreden DISH'li olgularda yutma guclugu ya da servikal miyelopati gorulebilmektedir. Posterior longitudinal ligamentin ossifikasyonuna bagli olarak miyelopati ile beraber bazen kuadripleji olabilmektedir. Osteofite bagli yutma guclugu ya da spinal darlik gibi ender komplikasyonlara bagli ameliyatlar nadiren gerekmektedir. Bu yazida 1.5 yildir yutma guclugu, boyun tutuklugu ve ust ekstremitede guc kaybi olan ve uc kez ameliyat gecirmis 62 yasinda DISH'li bir erkek hasta sunuldu. Tekrar ameliyatlara ragmen hastanin yakinmalari 2-3 ay sonra tekrarlamistir. Anahtar sozcukler: Diffuz idiopatik skeletal hiperostozis; yutma guclugu; ust ekstremite gucsuzlugu.
