Summary: | EarIy ROM exercise and early weight bearing following open reduction of the patella will prevent ankylosis of the knee joint and muscle atrophy, and enahle the patient to return earlier to their society. To achieve those goals authors tried to use load-sharing wiring with No 16 or 18 stainless steel wire or Dall Miles wire cable following open reduction and internal fixation of the patellar fracture with K-wires, screws, tension band wiring, and circumferential wiring, The results of the load-s4aring wiring group(IO knees) was compared with the conventional fixation group without load-sharing wiring(24 knees). The average follow- up period was 3.4 years ranging from 1 to 6 years. There were one late infection and resultant non-union of The fracture and two other non-union of the distal fracture in conventional fixation group. There was no such complication in load-sharing wiring group. Regaining of range of knee joint motion was delayed until 6 months or 1 year postoperatively in conventio- nal fixation group whereas ROM was normalized within 6 weeks postoperatively in load-sharing wiring group. Full weight bearing was achieved in 12 weeks postoperatively in conventional fixation group and in 4- 12 weeks postoperatively in load-sharing wiring group. From these results it was suggested that load sharing wiring was useful in early restoration of normal function following open reduction and internal fixation of the patellar fracture. |
