Summary: | Dear Editor, Diffuse panbronchiolitis (DPB) is a chronic inflammatory lung disease frequently complicated with chronic sinusitis, causing cough, sputum and respiratory distress. The Ministry of Health and Welfare of Japan (MHLW) proposes the definite diagnosis of DPB as having all of the 3 major criteria (1. persistent cough, sputum, and exertional dyspnea, 2. past history of recurrent chronic sinusitis, 3. bilateral diffuse small nodular shadows on plain chest X-ray film or centrilobular nodular shadows on chest CT images) and at least 2 of the 3 minor criteria (1. coarse crackles, 2. forced expiratory volume in 1 s (FEV1)/forced vital capacity (FVC) less than 70% and PaO2 less than 80 mmHg, 3. titre of cold agglutinin equal to or higher than 64). DPB in children are rare and some cases are firstly diagnosed as difficult-to-treat bronchial asthma. |
