Summary:Background: We studied the effects of amlodipine (A) and fosinopril (F) on heart rate variability (HRV) parameters and left ventricular mass (LVM) in essential hypertension (EH). Method: 27 patients with never treated mild to moderate EH were randomized to receive either F (10 or 20 mgr) or A (5 or 10 mgr) once daily as monotherapy. Obesity, diabetes mellitus, existing heart disease were the exclusion criteria. At baseline and at the end of the third and sixth month, each of the patients underwent time and frequency domain analyses of HRV obtained from 24 - h Holter recordings, 24-h noninvasive ambulatory systolic (SBP) and diastolic (DBP) blood pressure measurements were also recorded simultaneously. LVM index was calculated from echocardiographic examination at baseline and at the end of the 6th month. There were no differences at age, gender, body mass index between the groups. Results: In the A group (n=14), 24-h SBP/DBP (mmHg) decreased from 144 ± 8/94 ±4 to 128± 6/83 ± 3 at the end of the 3rd month (p
