description | This article aims a reflection on substitutive/alternative mental health services as a possibility of rehabilitation as well as promotion of citizenship concerning patients with mental disorders. The text provides an analysis, through relevant literature, of the institutional environment, from the psychiatric reform to the current status of the CAPS (Psychosocial Care Center) in the Brazilian Health System. It analyzes its creative process, its establishment as a practice favored by the government, its heterogeneity and its contradictions. The authors examine some actions taken by the teams working in the CAPS of Goinia and their approach to the ideals of the psychiatric reform. Pointing out the issues which have the greatest impact on the dynamics between patient and professional, they question the assumptions of the clinical care, its relationship to social practice emphasizing the value of singularity in attendance.
