DECREASED READMISSIONS AND IMPROVED QUALITY OF CARE WITH USE OF INEXPENSIVE CHECKLIST IN HEART FAILURE

Conclusion The use of inexpensive heart failure discharge checklist lead to higher proportion of patients being on the recommended medications, higher proportion of drug up titration and decreased 30-day and 6-month readmission rate.
