Summary:This is a case report about an AIDS patient with in situ cancer or penile intraepithelial carcinoma known as Bowen's disease. Penile Bowen's disease is usually caused by Human Papillomavirus (HPV), being serotype 6 the most frequently related to. The serotype of HPV in this case was not determined due to the inexistence of a proper diagnostic kit in the country. The treatment of choice for this entity is surgery, but the patient was diagnosed with HIV infection in the preoperative evaluation, being classified as C3 stage; therefore surgery was contraindicated due to severe immunosuppression related to low T-cell CD4+ count (154 cells per mL). For this reason, Imiquimod 5% cream was indicated with satisfactory results at six weeks. Recurrence after five months of follow-up is not observed.
