Summary: | For patients from the Western Cape transmission dynamics, infectiousness, and drug susceptibility were interrogated using whole genome sequencing (WGS), a cough aerosol sampling system (CASS), and phenotypic testing for 18 drugs, respectively. Findings: Overall, 203/273 (74%) XDR-TB patients had programmatically incurable TB and unfavorable outcomes of either treatment failure, relapse, default, or death despite treatment with a capreomycin and para-aminosalicylic-acid-based regimen. 172/273 (63%) of patients were home-discharged with 104/172 (60%) of these having an unfavourable outcome. 54/172 (31%) were treatment failures and their median post-discharge time to death was 9-9 months (IQR 4-2-17-4). |
