Summary:The theoretical and practical underpinnings of fees charged by mental health service providers are discussed in terms of historical and ethical contexts along with a selective review of published research. Key concepts are explained in the context of how they influence fee setting, discussion of fees with clients, fee disputes, and the influence of third-party payers. Practical considerations for dealing with these issues in the evolving health care system are discussed and recommendations for practice are presented. Clinical Impact Statement The issues and discussion contained in the manuscript will assist clinicians in considering their fee practices from both practical and ethical perspectives.
