Summary:An abstract of a study by Tanner et al examining transition processes from the perspectives of staff at the adult clinics who receive transitioning youth from adolescent clinics at 14 Adolescent Trials Network (MN) sites. Results showed that participants described transition as a "warm handoff" between the adolescent and adult clinics. Most adult clinics had no formal transition protocols (AAP Guideline #1) and many assumed that adolescent clinics would prepare youth for transition (AAP Guideline #2; e.g., life skills development, medication management, insurance documentation, budget). Despite the absence of formal protocols, many adult clinics worked closely with their adolescent clinic counterparts to create connections between adolescents and adult clinics (AAP Guideline #3). These collaborative strategies aimed to connect youth to adult clinics and facilitate successful transitions (e.g., youth site visits to adult clinics, adult providers conducting initial visits in the adolescent clinic, adolescent clinic staff accompanying youth to first adult clinic appointment). Finally, participants discussed the importance of the evaluation of the transition process (AAP Guideline #4), e.g.. staff identified necessary components (e.g., inter-clinic data sharing and communication) for evaluating transition outcomes (e.g., appointment adherence, viral load suppression).
