Summary:An abstract of a study by Holland et al using the Omaha System to describe patient problems and the care provided by a nurse in a pilot study of a technology-enhanced transitional palliative care (TPC) intervention with patients and caregivers transitioning from hospital to home is presented. The study results are the first description of a TPC nursing intervention using a standardized terminology. Linking interventions to patient problems is a critical first step in their ability to describe effective strategies in transitioning palliative care from hospital to home.
