A pear shaped bladder

At first glance, the pear shaped pelvic density (figure ; long arrow) resembles an incorrectly placed gonadal shield. Other differentials for this appearance include pelvic lipomatosis, vascular dilation/occlusion, enlarged pelvic lymph nodes, and psoas muscle hypertrophy.
