Summary:Individuals with expertise in relevant areas commented on 4 fundamental issues regarding Medicare and Medicaid: 1. impact on beneficiaries, 2. impact on health care providers, 3. impact on government, and 4. implications for the future. Public satisfaction with the 2 programs differs, partly due to differences in the beneficiary populations and in federal commitment. However, beneficiaries of both programs need to fight the threat of reduced benefits, and Medicaid beneficiaries are hindered by a lack of influence. Despite serious payment system defects, Medicare powerfully boosted the spread of new technology and the upgrading of hospital plant and equipment. Still, the government must be partly proactive in authorizing and financing health care programs and should be prepared to change the rules occasionally to respond to and provide incentives for change. At present, polls show public support for reform, and the programs fall far short of meeting the original objectives.
