Summary:Case A 77 years old female with coronary artery disease post myocardial infarction and mild vascular dementia underwent single lead AICD implantation for ventricular fibrillation cardiac arrest without reversible causes. Decision-making Twiddler's syndrome was suspected from recurrent early lead dislodgement and was confirmed by chest XR, although she denied manipulation of the device.
