Summary:Our office uses manila-colored patient charts, and occasionally a mix-up has occurred with charts of patients who have the same or similar names. However, since we started using red charts for those patients, we have had no episodes of pulling the wrong chart or misfiling examination results. A red chart alerts everyone--from the chart-puller to the physician--to double-check the chart.
