The presenting symptoms in this case patient already posed a challenge, because nausea and vomiting can be symptoms of both adrenal insufficiency and early cerebral edema. Because symptoms had been present for more than 2 days, the patient was judged to have chronic hyponatremia, and serum sodium was not corrected aggressively.
