To be successful in their interventions, nursing needs to meet the biological, social, psychological, and spiritual demands of the patient, providing a holistic view from their embracement at the unit until their discharge.8 The discharge plan requires the dedication of a multidisciplinary team, with the interaction taking place between all professionals involved in the health-disease process and to minimize the fragmentation of care.
