Summary:Objectives. To identify the clinical features of the course of temporal epilepsy with involvement of the insular cortex in the epileptic process. Materials and methods. A comparative analysis of diagnostic and therapeutic results from 55 patients with temporal epilepsy and 46 with temporal-plus epilepsy was performed. Neuroimaging, clinical, and electroencephalography data were compared. Results. Autonomic paroxysms in the form of nausea and hypersalivation, cutaneous manifestations, and simple motor seizures, along with emotional crises and sensory paroxysms, were the most significant signs of involvement of the insular lobe in the epileptic process in temporal epilepsy. Temporal epilepsy was characterized by cognitive paroxysms of the déjà vu type and auras in seizures. When the insular lobe was involved in the epileptic process, seizures were significantly more frequent, polymorphic, and structurally complex. Conclusions. Paroxysmal syndrome in temporal-plus epilepsy with involvement of the insular cortex showed significant changes in the form of increased seizure frequency and the occurrence of specific seizures.
