Here we present a case of 51 year old male with retroperitoneal swelling involving left kidney. Gross examination revealed a tumour that appeared arising from the renal cortex. Cut surface of which showed variegated appearance with yellowish areas and cystic changes mimicking renal cell carcinoma. Histopathologically it was diagnosed as high grade leiomyosarcoma infiltrating the adjacent renal cortex and the ureter. Immunohistochemically, the tumor cells were diffusely positive for smooth muscle actin (SMA). This is a rare case of retroperitoneal leiomyosarcoma with renal and ureteric infiltration.
