Summary:Purpose of Review The purpose of this paper is to review the current role of echocardiography in the context of ECLS therapy. Recent Finding Echocardiography plays a central role in the context of ECLS therapy with regard to the indication, implantation, and therapy management. Summary The pathomechanisms that lead to the development of cardiogenic shock are individual, the interaction of different influencesʼ complex, so that ECLS therapy remains challenging. Due to its constant availability, echocardiography plays a key role here. The focus should be on the daily determination of robust parameters with low inter-individual variability. The main focus of therapy control lies in the measurement of LV function, preload, mitral and aortic valve function, detection of LV overload, and early detection of complications. There are currently no evidence-based weaning algorithms; further research is needed here.
