Summary: | The patent ductus arteriosus (PDA) has long been a treatment dilemma in the field of neonatology. Standard treatment regimens have considerable side effect profiles and surgical treatment carries significant risk to the neonate. Recent studies have demonstrated that acetaminophen may be a safe and effective alternative treatment to indomethacin for PDA closure. Currently, there are very few studies that have examined safety and pharmacologic mechanisms of acetaminophen in premature infants. We conducted a prospective study of premature infants with a PDA that were treated with either indomethacin or acetaminophen. Urine was collected prior to initiation of treatment and at two subsequent time points while on treatment, day 2 and day 4. Since acetaminophen is postulated to produce its beneficial effects to close the PDA via inhibition of prostaglandin synthesis, urinary PGE metabolite levels were measured via an ELISA assay. |
