Summary: | Many community members have either no health insurance or difficulty accessing health care services and seek medical services, which can contribute to a higher risk of developing pressure injury in the perioperative setting. The Perioperative Shared Governance Committee implemented the following pressure injury prevention strategies and tools: * Reviewed the AORN standards, recommendations, and evidence-based practices. * Selected a standardized screening tool targeted to surgical patients. * Created a laminated Screening Tool with an itemized Pressure Injury Prevention (PIP) Bundle for use during patient assessment and preoperative planning. * Laminated positioning cards and made available in the OR as guide for nurses. * Converted all OR mattresses to a minimum of 3.5-inch thickness. * Purchased gel positioning aids. * Created Epic PIP smart phrases for each patient positioning. * Partnered with hospital's Wound Care Nurse to review PIP project and review retrospective data for one year. * Collaborate with Wound Care Nurse on educational program to incorporate proper skin assessment skills and documentation. * To roll out the PIP project one service per week with coaching. Standard tools established consistency for assessment and intervention focused on improving patient outcomes: * Collaborate with the nurse informatics manager to integrate the Scott Triggers Tool in Epic and populate the positioning bundles based on the Scott Triggers Tool. * Annual nursing competency and Orientation Program to include training of PIP tools and simulation. * Quarterly assessment of gel positioning aids. * Quarterly report of hospital-acquired pressure injury (HAPI) rates, and percentage of HAPI related to surgical cases. |
