Summary:We agree that Klein and Smith's (1999) vision for multisite efficacy/effectiveness intervention clinics possesses several advantages, but we ask whether these advantages may be had less ambitiously. Specifically, we suggest that if independent efficacy centers maintain their rigor and control but simultaneously broaden to examine an array of effectiveness concerns, and if the results from independent centers are capably pooled by meta-analysts, much of what Klein and Smith envision may be accomplished.
