Summary: | In this article, the authors describe a case of EPM with reversible extrapyramidal symptoms in a malnourished child following surgery for a craniopharyngioma. 2 Case report A six years old boy presented with history of headache and deterioration of vision since six months. FLAIR and T2 weighted sequences in a repeat MRI [POD 21] revealed symmetrical hyperintense signal changes involving the putamen, head of the caudate nucleus and thalamus, with sparing of the pallidum and brain stem (Fig. 1f-k). Malnourished patients with impaired regeneration of intracellular organic osmoles like myoinositol, taurine, glutamate etc. are vulnerable to the development of ODS after an acute osmotic shift [2]. |
