description Felodipine (F) is a new calcium antagonist agent with predominant action on vascular smooth muscle. Its efficacy in chronic heart failure has been demonstrated in previous studies. The aim of the present work was to assess the acute hemodynamic effect of F on regional blood flow in patients with moderate to severe heart failure. The results of this study indicate that in patients with chronic congestive heart failure (CHF), F has been able to induce an increase of cardiac output which at rest appears to be mainly distributed to the muscular vascular bed. Its failure to increase RBF is offset by an increased C sub(Na), probably secondary to a direct tubular mechanism shared by other calcium antagonists. As a consequence, the absence of sodium volume retention together with the decrease in afterload makes F a particularly beneficial agent in the treatment of CHF.
