Summary:Highlights • Empirical antibiotic treatment approach is accepted for infection diseases before cultures and antibiograms are available. • Oncologic patients requiring intensive care support are usually excluded from clinical trials. • Empirical erlotinib treatment can be reasonable in critical patients with lung cancer when an EGFR mutation is suspected. • This case report shows that erlotinib administration through nasograstic tube is effective in this setting. • Some clinically selected patients could get benefit from empirical erlotinib treatment as part of an ICU trial.
