Summary:We report on two patients with advanced Kienböck's disease (Lichtman IIIa and IIIb) who underwent surgical treatment in which the lunate was replaced by a pyrocarbon implant. At a mean follow-up of 3 years, the average VAS pain score was 1 and the average DASH was 11.5. There were no changes in range of motion after surgery and the average grip strength was 76.5% of the contralateral side. X-rays showed no disease progression. These encouraging preliminary results have led us to suggest replacing the lunate with a pyrocarbon implant in a greater number of patients with advanced Kienböck's disease.
