Use of an Itraconazole/Allopurinol Combination for the Treatment of Visceral Leishmaniasis in a Patient with AIDS

Summary: | We report the case of a patient with AIDS and visceral leishmaniasis who responded clinically to first-line treatment with a combination of allopurinol and itraconazole. A 32-year-old man who was infected with HIV-1 developed visceral leishmaniasis. Examination of a bone marrow aspirate and a blood smear revealed Leishmania species amastigotes. Culture of a bone marrow specimen yielded Leismania species. Concomitant fungal infection was ruled out by a thorough investigation (blood cultures, stains and cultures of bone marrow). The patient refused to be hospitalized; he was then treated on an outpatient basis with a combination of oral allopurinol (21 mg/[kg times d]) and itraconazole (400 mg once daily). Three months after therapy was begun the patient was free of symptoms, and his liver and spleen were no longer palpable. |
