Diagnostic value of lead aVR in acute coronary syndrome

In antreroapical ischemia, ST elevation is more prominent in V5 where as in inferoapical ischemia, ST elevation is more prominent in V7. Because V7 is not recorded in standard 12-lead ECGs, ST elevation secondary to inferoapical ischemia can be missed if ST depression in lead aVR is neglected.
