Summary:The patient was started on treatment for pneumonia, but a CT scan to rule out other causes showed a large incarcerated Morgagni hernia, characterised by a defect in the right anterior diaphragm with herniation of the omentum into the thorax, 22·6 cm at the widest point (figure). When otherwise well patients present with prolonged shortness of breath, or a right cardiophrenic angle lesion is seen on chest radiograph, physicians should consider the diagnosis of a diaphragmatic hernia.
