Summary:Highlights • DLNO /DLCO is a straightforward measurement of the ratio of the diffusing capacities of the lung for nitric oxide (NO) and carbon monoxide (CO). DLNO mostly reflects the Dm or membrane component and DLCO the θVc or red cell component. • The DLNO /DLCO ratio is positively related to the DmCO /Vc ratio in a curvilinear function, independent of the absolute values of DLNO or DLCO. • In clinical studies, a raised DLNO /DLCO ratio (≥110% predicted) is associated with pulmonary vascular disease, and a low DLNO /DLCO ratio (≤95%) accompanies interstitial lung disease with fibrosis.
