In the literature clinical presentation mimicking acute appendicitis has already been reported, thus a case of gallbladder volvulus in a parastomal hernia already exists,4 but to the best of our knowledge this is the first case of perforated gallbladder volvulus in a primitive Spigelian hernia, which, due to the hard right pararectal swelling that was very painful together with the acute abdomen clinical and laboratory picture, and unclear CT scan interpretation, was misdiagnosed as a strangulated Spigelian hernia also because the patient was presenting a very thin abdominal wall and a Spigelian hernia sac. |
