Summary:According to the patient, her swelling had increased in size over a few months before presentation.The differential diagnosis based on the imaging findings of the cortical arising tumour included modalities such as periosteal aneurysmal bone cyst, periosteal Ewing's sarcoma, periosteal osteosarcoma, periosteal chondroma, and—less likely, because of the patient's age—periosteal metastatic lesion (such as from primary lung cancer).The excised lesion was a 6·0 cm × 5·0 cm × 4·0 cm tan round firm mass with multiple blood-filled cystic spaces separated by tan-white fibrous septa of variable thickness, comprised of uniform spindled fibroblasts and occasional multinucleated osteoclast-like giant cells set in a collagenous stroma, characteristic of an aneurysmal bone cyst (figure, C).
