Summary:Cohen et al discuss two cases of antidepressant-resistant depression in patients with comorbid subclinical hypothyroidism or high-normal TSH levels. In case 1, a 35-year-old woman with a history of previous depression responsive to imipraminc at 150 mg/day presented with a second depressive episode that met criteria for major depressive disorder. In case 2, a 52-year-old woman had experienced episodes of depression in college and after the birth of her second child. These episodes responded well to fluoxetine and, later, escitalopram at 10 mg/day. Escitalopram was continued as maintenance treatment at 10 mg/day.
